Workers’ Compensation Care Coordinator

Our Workers’ Compensation Care Coordinator accepts assignments from the Clinical Care Manager regarding an injured worker. Once a case is referred, you will assess the clinical situation and determine the individual’s medical and behavioral health needs in conjunction with the Clinical Care Team. Through overseeing the case, your role is to facilitate maximum functional improvement for the patient. As a Workers’ Compensation Care Coordinator, your responsibility is to ensure that the patient is receiving quality care that is appropriate, necessary and in alignment with the Care Coordination Plan designed specifically to meet the needs of each patient. In doing so, you will also anticipate the patient’s future medical and behavioral health needs and aid in ensuring the proper mechanisms are put in place to meet maximum rehabilitation in the most efficient and cost-effective way possible. You will also assist in the development and presentation of Care Coordination Plans, working closely with Carisk’s experienced and strengths-based medical and behavioral health clinical team.

Job Responsibilities

  • Assesses and analyzes the injured worker’s medical, behavioral and/or vocational status, utilizing Carisk’s patient-centered, strengths-based biopsychosocial approach.

  • Assists in developing a Care Coordination plan to help the patient achieve maximum functional improvement.

  • Engages with the patient, current and potential providers as well as vocational and social support systems and other stakeholders externally.

  • Engages with Carisk’s Clinical, Quality, Provider Relations and other teams internally.

  • Manages a patient’s progress towards the desired outcomes.

  • Responsible for maintaining regular communication with the patient, medical providers, attorneys, employers, claims professionals, and Carisk teams.

  • Applies all special service instructions required by Carisk customers/accounts.

  • Appropriately applies all state jurisdictional guidelines, applicable laws and regulations to the provision of rehabilitative services.

  • Evaluates outcomes and patient progress towards the patients tailored Care Coordination Plan goals.

  • Prepares reports and other required documentation for casework activities.

  • Arranges referrals, consultations, therapeutic services, and confers with other specialists regarding course of care and treatment.

  • Substantiates case activities carried out/services rendered in all activity reports completed.

  • Accurately reports all case activity and hours associated in accordance with customer-specific guidelines and consistent with Carisk documentation practices.

  • Attends all meetings and case conference calls as requested by Carisk Clinical and Administrative teams.

  • Maintains any required credentials and adheres to all codes of ethics required by these credentials.

  • Performs various professional duties as assigned by management.

Job Qualifications

EDUCATION AND/OR EXPERIENCE

  • Current, active, and unrestricted licensure as a Licensed Clinical Social Worker (LCSW) or active and unrestricted Nursing License (RN or LPN)

  • 5 years case management experience required.

  • Workers’ compensation and/or Certified Case Manager (CCM) designation preferred.

OTHER DESIRED QUALIFICATIONS

  • Knowledge of the disability and workers’ compensation industry is ideal.

  • Practice case management within the scope of his/her licensure (based on the standards of the discipline).

  • Must be able to adequately operate a computer, fax, copier, scanner and telephone.

  • Must have strong knowledge of Internet use and multiple software programs, including but not limited to Microsoft Word, Outlook, Excel

  • Knowledge of current laws and regulations that govern delivery of rehabilitation services.

  • Knowledge of human behavior and performance.

  • Ability to demonstrate very strong, customer-centric knowledge, including needs-assessment, meeting quality standards for services and evaluation of customer/patient loyalty.

  • Must demonstrate exceptional communication skills by conveying necessary information accurately, listening effectively and asking questions where clarification is needed.

  • Must be able to work independently, prioritize work activities and use time efficiently.

  • Ability to concentrate and multitask in a fast-paced work environment.

  • Demonstrates accuracy and thoroughness. Looks for ways to improve and promote quality and monitors own work to ensure quality is met.

  • Must be able to maintain strict confidentiality.

  • Must be able to demonstrate and promote a positive team-oriented environment consistent with Carisk’s Guiding Principles.

  • Must be able to work well under pressure and/or stressful conditions.

  • Must possess the ability to manage change, delays, or unexpected events appropriately.

  • Must possess a valid driver’s license.

  • Must possess excellent skills in English usage, grammar, punctuation and style.

  • Demonstrates reliability and abides by the Carisk company attendance policy.

  • Must maintain a professional and clean appearance at all times consistent with Carisk company standards.
